Vital Records - Birth and Death Certificates
Services:
- To register all birth and deaths occurring in Jefferson County
- To maintain accurate birth and death register
- To assure all vital records are complete accurately
- To prepare certified copies to the public upon request
- To amend original birth and death records when needed
- To be available to the public for notarizing paternity affidavits when needed
- To keep abreast of all state rules and regulations
- To assist local hospital and funeral homes with questions regarding records
- To maintain a good working relationship with the State of Oregon Vital records office
- To forward all original records to the State of Oregon office in a timely manner
- To keep copies of the original record for at least 6 months for certified copies
You may order a certified copy of a Birth or Death Certificate in person for up to 6 months following the event.
- You will need to fill out a short form.
- You will need a picture identification card (ex: driver's license, state issued identification card)
- The cost for all certified vital records is $25.00
- To order by mail see the links below for the Jefferson County Public Health Vital Records order form. Include a check payable to Jefferson County Public Health, and mail to Jefferson County Public Health.
- Please allow at least one week for delivery.
To order a record for an event older than 6 months old, please contact the State of Oregon Vital Records office.
